Kindergarten Registration Check List

For Student File:

   _______ Eye Exam from a Certified Ophthalmologist (state mandated)

   _______ Original Baptism certificate (Catholic)

   _______ Copy of Student Social Security Card

   _______ Updated school physical form from the doctor (6th graders only)
